The following are cash and insurance billing fees:

 

Initial Evaluation - $200

30-minute sessions - $100

45-minute sessions - $150

60-minute sessions - $175

Not covered by insurance:

 

Missed Appointment - $100

Late Cancellation - $75  (after 8 am day of appointment)

Telephone Consultation - $75 for 25 minutes to $150 for 45 minutes

Documentation, Letters and Reports – $50 and up, time spent
